Body Piercing of minors is permitted only with written, notarized consent of at least one parent, or legal guardian. If you are under the age of 18 and would like to be pierced, please print the appropriate form below and have it notarized prior to your arrival at the studio.
Piercings performed solely upon the earlobe are permitted at any age.
